High Holy Days - Yizkor Remembrance Order Form

Our Book of Remembrance is a wonderful tradition.  It is distributed at the Yizkor Service and provides us with an opportunity to keep the memories of our departed loved ones close to us during the Holiday season.  The book will automatically include the names of CBT members and relatives who have passed away since last Yom Kippur.   You do not need to do anything for the names of the individuals falling under this category to be listed in the Book of Remembrance.

If you would like to include a listing for anyone else, please complete and submit the form below. A contribution of $18 per name is requested.
